Steed OT is committed to ensuring both the privacy and accuracy of your personal and health information that we collect.

Collecting and Storing your Information

Information collected by Steed OT complies with the Privacy Act (1988). Steed OT collects and holds information that is reasonably necessary for providing and evaluating services and as otherwise permitted by law. This may include your personal information (eg name, date of birth, address, contact details), health information and other relevant information.

We may collect this information directly from yourself or a third party that may assist in providing services eg a carer or another health care provider.

Information collected by Steed OT is stored on a secure electronic practice management software. Sometimes hard copy documents may be required and these are also stored securely. The security of your information is important to us and we take reasonable measures to protect your personal information from unauthorised access, misuse, damage and loss.

Accessing and Disclosing your Information

You may access the information we have retained about you as per the Privacy Act (1998). Requests can be made in writing to Steed OT. Sometimes, requests may be declined if giving access would unreasonably impact on someone else's privacy or the safety of yourself or another person.

We may disclose information to third parties as reasonably necessary for the purposes of providing you with services and if permitted by law. These purposes include:

  • communicating relevant information with carers and other treating professionals eg general practitioners, private psychiatrists, NDIS support coordinators and other health professionals for the purpose of health service provision

  • for follow up reminders and information related to your appointments

  • auditing, accounting, medicare/insurance activities

  • out of a duty of care, if we form a professional opinion that disclosure is necessary to prevent or lessen a threat to the life or wellbeing of yourself or another person

  • when required by law

  • in clinical supervision (identifying details are not disclosed)

Steed OT does not usually disclose your information overseas.

Correcting information

Steed OT will take reasonable measures to ensure your information is accurate and up-to-date. However, if you feel this is not the case, you may contact us in writing to request a change to this information. If we believe that the information we have is correct, we may note your view in your records.

Feedback/Complaints

Steed OT endeavours to work collaboratively, respectfully and to a high standard. Feedback (positive and negative) helps us to deliver high quality services and make improvements and we take feedback seriously. If you are unhappy with an aspect of our service, Steed OT requests that you contact us to hopefully resolve any concerns.
